Contact about a new appointment or to reschedule an existing appointment

not all scripts can be renewed outside of appointments. If we are unable to renew your script outside the appointment, we will contact you by email advising so.
renewal of scripts will incur an administration cost unless you are within two weeks of completing your appointment
when submitting the request for script renewal either by email or in the message box below, we request you to provide us

The name of your pharmacy
Fax or email address of your pharmacy where the script will be faxed
Postal address of your pharmacy where the script will be posted
The medication that you wish to be renewed
The dosage of the medication that you wish to be renewed

Please note that without these five items in the message for a script renewal, your request may not be able to be completed. We urge patients to ensure you have enough supply of medications and have your scripts renewed during your appointment.
